Ziya Saylan , MD ; Dusseldorf , Germany An internal support will maintain an ideal postoperative breast shape and anterior projection which also permits a long lasting support and projection of the breast by counteracting gravity. We have developed a technique to implant cups of titanium mesh under the breasts. These are then attached to the patient's ribs and muscle. The effect is claimed to be instant, youthful-looking breasts with invisible lift. Since almost 2 years he inserts an internal BRA out of titanized polypropylene- a mixed mesh which is called TiMesh ® (GfE Medizin Technik, Nürnberg, Germany; Boss Instruments, Nashville , TN ) and used mainly in inguinal and abdominal hernia repair which shows no foreign body reactions in compare to other mesh grafts. This so called internal titanium bra will be suspended to the pectoralis muscle, sternal bone and to the ripcage. Since March, 2003, more than three dozen women have undergone this procedure.In our opinion "The advantage is that a woman with these implants will almost never have to wear a bra again". Titanium is flexible but extremely strong. It is completely inert, so will not cause an allergic reaction in the body, and the breasts will never sag. The procedure can be done at the same time as a breast augmentation with an implant to increase the size of the breast or during a vertical mastopexy or periareolar breast reduction. It is performed under twilight anesthesia or general anaesthetic in a sitting position.
Titanium Bra can be also used against sagging breast implants! Most of our patients are women of between 30 and 48 who have developed sagging breasts after childbirth or weight loss. Older patients are less suited for the procedure because their skin may have lost some of its elasticity. We believe that this is something that is better for younger patients. At the start of the surgery, the patient is placed in a sitting position so the breasts hang down over the chest. A semicircular cut is made around each nipple and space is made for the titanium mash between the glandular tissue of the breast and the subcutameous fat just under the skin. It is important not to damage the fat layer, because otherwise the implant may show through. The titanium mesh (Timesh®, GfE Inc. Nurnberg, Germany), which weighs just 16g and is as thin as net curtains, is cut to size and moulded into a cup shape. It is flexible enough to be rolled up and inserted through the small incision round the nipple and then manoeuvred gently into place. The implant is then held in place by stitches that attach it to the breastbone and the pectoral muscles. The sutures dissolve after ten to twelve weeks, when it has already fused with surrounding tissue. Patients stay in hospital overnight but can be back at work after a week. "At first, the breast is tender to touch, just like after a sports injury. After a short while, however, you will not even know it is there.

Afterwards, the breasts are higher and firmer, because the tissue is compacted into the titanium cup. Apart from the cosmetic effect, patients who go bra-less may get health benefits, too. A U.S. survey of 4,700 women showed the odds of getting breast cancer increased dramatically the longer women wore bras. The study concluded that the constricting effect of bras suppressed the lymphatic system below the armpits, blocking a network of vessels supposed to flush toxic wastes from the body. Dr Saylan's patients have all been happy with their surgery, but he advises caution. "Titanium is not biodegradable and stays in place permanently. It will be difficult to remove if things go wrong. Women may gain weight or change body shape as they get older. "A titanium bra cannot be swopped for a D cup." The titanium bra's could also make it more difficult for women to be breast aware. Dr Saylan admits the titanum net makes it more difficult to palpate and to detect breast tumors. "The ladies have to be extra careful. We are concerned there is a danger that they may miss tumours which need to be spotted early to maximise the chance of a patient's survival under the mesh so we always emphasise the need to have regular mammograms and ultrasound examinations."
